Living Well Homecare is looking for a Care Coordinator in the Stevenage area.

Living Well Homecare is a domiciliary and community care provider, we support service users with their day-to-day needs. Our team also supports adults with mental health and learning disabilities.

  • Collaborate with service users, their care circle, and healthcare professionals to ensure a clear understanding and delivery of quality care.
  • Assist the care coordinator in planning effective support worker rotas, matching worker skills to service user needs.
  • Conduct initial assessments and regular reviews of care packages, updating needs and risk assessments as necessary.
  • Provide support and guidance to support workers, ensuring their understanding of service users' needs and promoting safe working environments.
  • Record and escalate concerns, complaints, and compliments appropriately, and effectively share information with relevant professionals.
